Detoxification Unit - Intake Guidelines

You've taken an important first step in your recovery process by asking for information on Main Street Project's detoxification unit. The following will clarify our intake process, and our expectations of all clients who are accepted into this unit. Please review all attached information and return the complete document, signed, when presenting for intake. If you have questions while preparing for your intake, please ask any member of our staff, or call us directly at 982-8245.

Information available on this page includes:

Intake Process:

1. All clients must provide a completed MSP medical clearance form at the time of their intake. This form should be completed by your regular physician, where possible, and must include:

  • All general client and physician information at the top of the form;
  • A listing of all existing medical issues;
  • A listing of known medical issues that are no longer a concern;
  • A listing of all required medications, and what they are for;
  • A listing of medications on file with MSP, but no longer required (if you have used MSP services in the past, please ask if we have medications on file that need to be addressed, as this can have a significant impact on your intake process);
  • Any additional comments your physician feels are relevant to your intake.

MSP medical clearance forms are only valid for 72 hours after completion by your physician.

2. Clients must present with sufficient medications, as outlined by your physician in the medical clearance form, for the entirety of your stay at MSP. All medications must be current, in appropriately labeled containers, and include clearly outlined dosage instructions. Clients presenting without necessary medications or with medications that do not meet the guidelines above will be denied access to the detox unit until those issues can be corrected.

3. Clients must present with only those personal items that are listed on the attached guideline for what you are allowed to bring into the MSP detox unit. There are no exceptions, and as we do not have room available for storage, those arriving with additional items will not be allowed access into the unit. Please make suitable arrangements for your items prior to arrival.

4. Clients must review, and accept the house rules outlined in this document. The rules are written to ensure everyone an equal opportunity to heal, discover, and move forward in recovery.

5. Clients must review and sign this document, indicating understanding and acceptance of all MSP detox guidelines. If you have any questions, please ask before your intake.

6. Once you have presented with all required information, a full intake appointment will be arranged. This meeting will help us to assess how our services can be of value to you. No intake is final until this meeting has been completed, and the intake approved. Intake appointments must be made in person, and cannot be accommodated over the phone.
